Dr. Sam received her Associates Degree in dental hygiene in 1993. She received her Doctorate in Dental Surgery in 2000 from the Indiana University School of Dentistry and received her Masters of Science and Certificate in Orthodontics in 2005.

While in school, she received the Garner Minority Scholarship, Quintessence Clinical Achievement Award, Periodontal Clinical Award, National Dental Association Colgate-Palmolive Academic Scholarship and was on the Dean’s List.

After practicing in Fishers for several years, Dr. Sam chose to open her own practice in 2007. Our newest office is located near St. Vincent’s Women’s Hospital and proudly serves patients from Indianapolis, Zionsville, Carmel, Fishers and other locations in Central Indiana.

Dr. Sam is married to Dr. Michael.  They live in Indianapolis with their children. She reading, traveling and most of all, spending time with her family and friends.
